Transition Hub
AfC Transition Hub assists Children Looked After aged 11-16 years old, by providing at least 6 weeks of bespoke support with their transition into care or to a new school or foster care placement. Students are automatically referred into the Hub by AfC Virtual School staff.
AfC Transition Hub also supports children who are the subject of a Child in Need or Child Protection Plan. Students aged 5-16 years old with a social worker, where there are concerns regarding their school attendance, can be referred into the Transition Hub by their social worker or Designated Safeguarding Lead.
The support for Children Looked After is bespoke. It will include subject specific tutoring, pastoral support, Motivational Interviewing and an educational outing to support their learning.
The support for Child In Need and Child Protection students will be needs based, with decisions on appropriate and targeted interventions being made on a case by case basis. We offer pre teaching, catch up learning, exam preparation, support in class and Motivational Interviewing.
Alongside our student offer, we also provide support to foster carers, parents, social workers and schools. This could be in the form of training, visits, consultations, advice, guidance and signposting to other AfC services and external organisations.
